Comprehensive Health History for Metabolic Assessment (Lab-based work-ups)  

This form is an indepth health history geared for patients with underlying metabolic health conditions/concerns.  For example, unknown reasons for fatigue, despite "normal" medical lab results. 

The Comprehensive Health History, along with copies of all medical records, must be received prior to scheduling an appointment.
